In consideration of the services rendered by Friends of Rosebud to my dog(s), I acknowledge reading, understanding, and accepting the statements herein. Please acknowledge the following policy statements by signing below. 

 

Liability 

 

Exposure to parasites, viruses, and other medical conditions passed from dog-to-dog or person-to-dog; sprains, strains, bites, broken bones; motor vehicle accident during transportation; and fatigue, dehydration, nicks, cuts, or death.

I further understand that not each and every potential risk can be listed above but, nonetheless agree that the benefits associated with dog socialization outweigh the possible risks, therefore, I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Friends of Rosebud and its agents, successors, heirs, from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my dog’s participation in activities with Friends of Rosebud.

Further, I understand that I may be exposed to certain risks when bringing my dog to participate in activities at Friends of Rosebud or when picking up my dog from participating in activities at Friends of Rosebud. Such risks may include property damage and/or physical injury inside or outside, such as from falling, slipping, illness, and/ or dog bites. Therefore, I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Friends of Rosebud, its agents, successors, heirs from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my dog’s participation in activities with Friends of Rosebud.

Authorization of Medical Care

 

If my dog is ill or injured while participating in activities at or with Friends of Rosebud, Friends of Rosebud will make every reasonable effort to reach me pursuant to the contact information I have provided Friends of Rosebud. However, if Friends of Rosebud is unable to reach me, I consent to Friends of Rosebud seeking appropriate veterinary care and I accept responsibility for any and all associated expenses. Friends of Rosebud will not pay any portion of veterinary expenses associates with seeking medical care for my dog if so necessary. 

 

Allergies, Special Diets, Medications

I agree that I will disclose to Friends of Rosebud any allergies my dog may have. I further agree to disclose to Friends of Rosebud any special dietary needs or medications my dog may require if necessary during activities with Friends of Rosebud.

 

Agreement to Pay

​

I agree to pay the service rates in effect for my dog’s participation in activities at or with Friends of Rosebud. 

​

Damage

​

I accept the responsibility of paying for any damage to property, and/or equipment caused by my dog.

​

Right to Decline

​

I understand that Friends of Rosebud reserves the exclusive right to decline participation or to terminate participation in activities at Friends of Rosebud to any dog at any time for any reason.

 

Attorney Fees Applicable by Law and Venue

​

Should Friends of Rosebud, or anyone acting on their behalf, be required for any reason to incur attorney fees and costs to enforce or defend this agreement, I agree to indemnify and reimburse Friends of Rosebud for such fees and costs. Further, I agree and understand that any disputes arising out of this Agreement will be decided pursuant to the laws of the State of Massachusetts and the venue shall be in Middlesex County.

Valid Dates

​

These agreements, waivers, and authorizations will remain valid and in force as long as and whenever my dog participates in any activity at or with Friends of Rosebud.

 

Warning

​

By signing this document, I acknowledge that if my dog is or I am hurt or property is damaged during my dog’s participation activities with Friends of Rosebud, I may be found, by a court of law, to have waived my right to maintain a lawsuit against Friends of Rosebud on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read and fully understand this entire document and I agree to be legally bound by its terms.
